Job Description:

Become a vital part of our compassionate community as a Home Health Registered Nurse and help us prioritize health. We are offering a $20,000 sign-on bonus for this position. Responsibilities:

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management, coordinating with healthcare personnel and reporting patient progress.
• Confer with physician to develop initial plan of treatment based on patient assessment and physician's orders.
• Coordinate/oversee/supervise the work of other healthcare providers and provide written instructions/care plans.
• Prepare appropriate medical documentation on all patients and confer with other healthcare disciplines. Requirements:
  • Diploma, Associate or Bachelor Degree in Nursing
  • Minimum of one year nursing experience preferred
  • Current and unrestricted Registered Nurse licensure
  • Strong organizational and communication skills
